HEAL for Reentry Case Manager

Tacoma, WA

 

Responsible for helping individuals returning from incarceration back into the community to gain access to programs and community services. Duties include meeting with clients, connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their reentry plan as needed. Those with lived experience in the criminal justice system are encouraged to apply.

 

Native American Reentry Services is a 501c3 nonprofit organization. Our mission is to address barriers faced by Native people struggling to transition from a life of incarceration to one of healthy and productive freedom. We support successful reintegration into Native American/Aboriginal reservation and urban communities and reduce recidivism through Native American/Aboriginal traditional means.

 

Case management encourages participation in cultural and spiritual activities and use peer education and interventions, assisting clients in addressing barriers faced in transition from a life of incarceration to one of healthy and productive freedom.

Experience

Case Management

  • Able to screen, conduct intakes, and document client information and communications.

  • Developing, and monitoring individual service plans for residents navigating client services, navigate housing uncertainties for clients facing mental health challenges

  • Monitor client progress & revise to ensure goal achievement

  • Provides assistance in crisis management, overcoming obstacles to goals with regard to education, employment, housing and personal issues

  • Motivational interviewing with empathy, patience, and concise language in crisis situations

 Social Services

  • Makes referrals to other services as needed

  • Follow up to ensure services are provided appropriately

  • Support completion of assistance program applications

  • Plan development, action steps with clients and social workers 

  • Identify, obtain client benefits and services

  • Respond to requests to assist accessing resources, services, court systems

  • Follow up to ensure receipt of intended service, meeting client needs

 Cultural/Community Building

  • Coordinating with NARS staff in scheduling events and meetings

  • Responds to general public, tribal stakeholder inquiries

Group Facilitation

  • Experience or knowledge about Wellbriety, group facilitation

 Demonstrated ability to work with incarcerated Native American and Indigenous individuals and their communities; aptitude for working effectively with an interest in the reentry needs of Iron House Medicine and HEAL for Reentry program participants; demonstrate emotional stability; adaptability; firmness; patience; self-control; tact; good communication skills; good judgment in moral, ethical, and spiritual matters; ability to independently travel to prison sites statewide

 Possess a valid Washington State Driver’s license and have proof of auto insurance.

 Desired Education & Experience Desired

·       1-3 years of professional work experience providing case management

·       Bachelor’s Degree in Social Work, Administration, Human Services or related field

·       or Associate’s Degree in Education, Administration, Human Services or related field

·       or High School Diploma/GED Completion and four (4) years of professional work experience providing case management.
